    Essential Responsibilities:

    + Be responsible and accountable for resident care, staff and the environment of care.

    + Ensures thorough and prompt reporting of incidents with follow-up as necessary to the DON and/or Administrator.

    + Encourage each resident’s ability to maintain or attain the highest practical physical, mental, and psychological well being and thus performs various duties to provide quality nursing care to residents and to coordinate total nursing care for the residents as illustrated by the following:

    + Assess residents and analyze obtained information to develop appropriate interventions to prevent decline; record signs of change in condition, notify appropriate staff per facility protocol to evaluate weight loss, decline in ambulation, change in performance of ADL’s, development of skin breakdown, etc.; observe conditions which indicate possible need for restorative nursing programs and then notify appropriate staff per facility protocol of the need for evaluation; inform the physician of changes in assessment when appropriate.

    + Assess the resident upon admission and develop interim care plan, contribute to the full care plan, and update the care plan as warranted and as required; attend and participate at the Plan of Care meetings as scheduled.

    + Administer medications and treatments to residents as needed; monitor the administration of intravenous fluids as needed; place orders for medications from the pharmacy.

    + Perform other duties as required.

    UPMC Home Health is looking to hire a Child Development Specialist to provide in-home services to children and families in Early Intervention. This position focuses on family coaching to help parents/families to support the early developmental needs of their child.

    This position is part-time Monday-Friday between 8:00 am-7:00 pm, scheduling patients is at the employee's discretion. Services provided in Blair County with the possibility of services in Centre, Bedford/Somerset counties.

    **Responsibilities:**

    + Demonstrates and teaches activities to the parents or caregiver to incorporate into typical family routines and play.

    + Evaluates enrolled children on an annual basis and reports results to Multi-disciplinary Team to determine continued eligibility for service.

    + Endeavors to achieve and maintain quality and customer satisfaction in the delivery of services to internal/external customers.

    + Completes and submits all required documentation (i.e. service plan, quaterly/6 month/ annual review, contact logs progress reports, etc.) within the timeframes dictated by regulations, county/Agency processes and best practices.

    + Responds to families needs for special services and refers to Service Coordinator when appropriate.

    + Collaborates with agencies involved with the family (educational, social services, consultants, etc.) to provide maximum effort toward a comprehensive delivery of service.

    + Meets or exceeds EI productivity standards as well as adheres to other budget standards related to expense and revenue.

    + Plan strategies and activities.

    + Consults with therapist(s) and other team members on individual child-related activities to assure consistent delivery of service.

    + Ensures staff and customer safety by resolving and/or reporting all safety issues and injuries to the Director.

    + Collaborates with the family to develop a description of their child and their Early

    + Bachelor's Degree in Early Childhood Education, Special Education, Child Development or related field. Teaching Certificate I preferred.

    + Prior experience working with children is preferred.

    + Competency in computer skills in a Windows based environment. Demonstrate ability to perform computer skills in specialized computer application(s) as needed.
