A Licensed Practical Nurse (LPN) administers nursing care to designated patients under the direction and supervision of a registered nurse. This is a full time (40 hours/wk) position.

Shift: 7PM – 7AM | Every other weekend and holiday rotation

**Responsibilities:**

+ Observes and records resident's symptoms, condition, and/or reactions to medications and treatments and reports results to the RN in charge.

+ Strictly adheres to all policies and practices relative to resident care and facility-related information.

+ Prior to performing new or challenging skills, requests assistance and reviews relevant policies and procedures.

+ Assists the RN and interdisciplinary team in evaluating, planning, and implementing plans of care.

+ Suggests and supports changes within the department.

+ Discusses any resident or family concerns with RN in charge, so that issues can be appropriately addressed.

+ Provides appropriate, direct resident care. Gives injections, takes vital signs, performs basic diagnostic tests, observes residents, dresses wounds, administers medication. Provides IV therapy as permitted by practice act and policy.

+ Contributes to the unit activities including, but not limited to, responding to call lights and telephones, passing meal trays and transporting residents.

+ Takes action to improve knowledge, skills and performance based on feedback or from self-identified development needs.

+ Oversees care given by Certified Nursing Assistants to ensure adequate and timely care is provided .

+ Establishes professional and respectful interpersonal relationships.

+ Identifies and communicates learning needs to the Unit Manager/Nursing Leader.

Completion of a Practical Nursing Program approved by the State Board of Nursing. Long term care experience preferred.

CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

Work Schedule: 10 or 12 hour days. No on-call.

The Peri-anesthesia Unit is made of tree departments; Preop, PACU and phase 2. In Preop we admit patients for surgery with detail given to current health status. In PACU we receive the patient from the OR and are focused on recovering them from anesthesia with particular emphasis on their airway. In phase 2 we ensure the patient is safe to go home and provide discharge instructions and education.

The Licensed Practical Nurse (LPN) Act as a member of the health-care team by exercising sound nursing judgment based on preparation, knowledge, experience in nursing and competency and able to provide care and IV therapy following UPMC LPN Practice Guidelines and policies, and the State Board Regulations. The Licensed Practical Nurse (LPN) participates in the planning, implementation and evaluation of nursing care using focused assessment in settings where nursing takes place under the direction of a Registered Nurse or other licensed UPMC provider. A senior LPN is responsible for selecting and completing 2 contributions annually that will assist in improving patient care OR must have and maintain a UPMC approved certification and also select and complete 1 contribution. The role of the Senior LPN encompasses partnership, and collaboration with the health care team and the patients and families to achieve desired patient outcomes.

**Responsibilities:**

+ The LPN is licensed to function as a member of the health-care team by exercising sound nursing judgment based on preparation, knowledge, experience in nursing and competency. The LPN participates in the planning, implementation and evaluation of nursing care using focused assessment in settings where nursing takes place.

+ An LPN shall communicate with a licensed professional nurse and the patient's health care team members to seek guidance when The patient's care needs exceed the licensed practical nursing scope of practice. The patients care needs surpass the LPNs knowledge, skill or ability. The patients' condition deteriorates or there is a significant change in condition, the patient is not responding to therapy, the patient becomes unstable or the patient needs immediate assistance.

+ An LPN shall obtain instruction and supervision if implementing new or unfamiliar nursing practices or procedures. An LPN shall follow the written, established policies and procedures of UPMC, UPMC LPN Practice Guidelines, and the State Practice Act. Acts within the LPN Practice guidelines including IVs and within the framework of Relationship Based Care to create a healing environment. Formulates daily goals and a plan of care for patients that involves the patient as partner and considers the individual needs of the patient in a holistic manner.

+ Communicates the patient's condition to other care providers. Patient documentation is comprehensive and promotes communication between caregivers. Incorporates national professional organization as well as business unit and health system's goals to improve patient safety, quality and satisfaction of the patient experience in daily work. Creates a caring and compassionate patient focused experience by building healing relationships with patients, families and colleagues.

+ Actively participates in unit activities and willing to provide assistance to care team members. Provides detailed and appropriate teaching to patients and families to effectively guide them through the episode of care as well as transition to another level of care and/or home. Demonstrates a passion for developing others by supporting the orientation and developing new LPNs and patient care support staff. Regularly seeks feedback on performance and applies continuous professional improvement.

+ Supports continuous learning in the clinical areas by participating in organizational and system programs. Identifies opportunities for clinical quality as well as workplace improvement. Participates in the development, revision and implementation of policies and procedures designed to insure comfort and safety of patients in collaboration with other health care personnel. Seeks and participates in opportunities to assist in identification process improvement opportunities to improve patient care.

+ Develops and maintains productive working relationships internally and externally by demonstrating accountability for actions, enthusiasm, motivation and commitment to patients and colleagues. + Successful completion and graduation from a Licensed Practical Nursing Program approved by the State Board of Nursing. Successful completion of the IV therapy curriculum.

+ Minimum 2 years of experience. If graduated in 2012 to present day, IV therapy curriculum is included in the PA State Board Approved Programs. If graduated prior to 2012, the LPN will need to provide official proof of completion of IV therapy approved program. Must select and complete 2 contributions annually that will assist in improving patient care OR must have and maintain a UPMC approved certification and also select and complete 1 contribution.

+ The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below.

+ They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures.

+ Ability to identify process improvement opportunities to improve patient care and encompasses collaboration with the health care team and the patients and families to achieve desired patient outcomes. Participates in the development, revision and implementation of policies and procedures designed to insure comfort and safety of patients in collaboration with other health care personnel.

+ Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.

+ Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment.

Summary:

+ The Licensed Practical Nurse (LPN) functions collaboratively within the team by observing the patient/family needs and facilitating care. What you will be doing?

The Nurse is responsible for promoting a safe and therapeutic environment for clients all clients of BriteLife Recovery. At BriteLife Recovery, we offer a unique opportunity for nurses to maintain their therapeutic relationships with their patients without the chaos and unsafe staffing ratios that are common in the hospital setting. There are no bedpans, IVs, or bed alarms and all clients perform their own ADLs.

What tasks are required?

+ Assists clinical team with the Admissions process;

+ Administers medications, treatments and procedures according to standard nursing practices and applicable facility policies;

+ Carries out a plan of care and medication orders from Physicians and Nurse Practitioners;

+ Educates clients regarding potential side effects of medications and observes for side effects;

+ Monitors clients for substance misuse related withdrawal symptoms;

Licensed Practical Nurse-Outpatient Float - Occupational Health Carlisle Street

General Summary

Provides safe, therapeutic and efficient care and services to patients. Functions as a member of the health care team at an intermediate nursing level to assist in the planning and implementation of patient care based upon sound nursing judgment, clinical preparation, knowledge, skills and experience. Nursing care is provided in accordance with the provisions of the Pennsylvania State Board of Nursing. Will perform a variety of clerical office duties related to telephone management, receiving patients, and scheduling. Will have the ability to float to Medical Group practices as assigned.

Duties and Responsibilities

Essential Functions:

+ In collaboration with physician, provides extensive health education, counseling and advice to patients and families on topics such as life style changes, cholesterol lowering diets, diabetic control etc. Effectively assesses patient educational needs, prioritizes patient's learning needs, identifies barriers to learning, develops teaching plans, and uses various educational tools to activate the non-compliant patient.

+ Tracks, reviews and monitors follow-up of diagnostic test results, under the direction of the provider. Provides related education, counseling, ongoing review and monitoring regarding diagnostic tests (i.e., INR results in Anticoagulation Clinic) in collaboration with the provider. Assists in the management of patient care between visits. (i.e., prenatal care).

+ Following defined protocols independently assesses patients/callers health status based upon presenting symptoms. Determines the urgency for care and directs patient/caller to most appropriate level of care, or an acceptable alternative venue for care of patients with acute problems should access not be readily available at the practice. Seeks provider advice appropriately.

+ Performs the responsibilities of the Physician Office Clinical Assistant as outlined below:

+ Greets and registers patients in accordance with established policies and procedures.

+ Schedules patient appointments within established parameters.

+ Schedules patient appointments for patients with other provider offices including correct processing of necessary referral forms, pre-authorization forms, test instructions, patient benefit verification and transfer of patient information.

+ Maintains patient records to ensure an accurate record of the patient's medical treatment.

+ Under the direct supervision of a licensed nurse or provider, prepares and administers vaccines, medications and controlled substances/narcotics as prescribed by the provider and as certified competencies permit. Asks prescribed screening questions, identifies risks and contraindications, questions and observes for appropriate responses.

+ Documents and processes prescription requests and refills per provider order.

+ Performs basic office procedures, examinations and/or tests ordered by provider and as certified competent (i.e. EKGs, ear lavage, vision tests etc.).

+ Provides health care information materials and reviews with patient and family members as directed by providers (i.e. VIS sheets etc.).

+ Under the direction of the provider, tracks, reviews and monitors follow-up of diagnostic test results. Relays information to patient from the provider regarding results and follow-up instructions.

+ Obtains basic health information from patient/family members to review with providers or clinical resource person to determine the urgency for care or an alternative venue for care for patients with acute problems, should access not be readily available at the practice. Gives basic care information to patients/family members per provider guidelines and instructions.

+ Obtains and documents reason for visit, patient vital signs, including temperature, blood pressure, weight, respirations, pulse and BMI.

+ Assists providers with examinations, procedures and other processes related to direct patient care.

+ Collects specimens as directed by provider. Uses appropriate equipment and protocols for collecting, handling, testing, and preparing for transportation to lab including proper identification, storage procedures and techniques.

+ Assures all patient care areas are adequately stocked with unexpired supplies and medications.

+ Prepares patients for scheduled procedures by assuring patient is appropriately gowned and instruments and supplies are set-up. Uses Universal Precautions. Sterilizes instruments, equipment and supplies as required. Handles and stores instruments, equipment and supplies appropriately.

+ Arranges for patients' ordered diagnostic and therapeutic services by completing necessary paperwork and scheduling appointments.

Nursing care is provided in accordance with the provisions of the Pennsylvania State Board of Nursing. In addition, serves as a nursing resource for clinical staff.

Duties and Responsibilities

Essential Functions:

+ Obtains patient history and gathers information.

+ Performs specialized procedures, examinations and/or tests as ordered and aligned with the Ambulatory Clinical Scope Document, such as, urinary catheterization, and insertion of intravenous lines (IV) with an approved IV certification training.

+ Ensures all patient care areas are adequately stocked with unexpired supplies and medications.

+ Sterilizes instruments, equipment, and supplies as required, when applicable. Handles and stores instruments, equipment and supplies appropriately. Assesses sterility of instruments, ensures they are intact, and supplies are set-up.

+ Relays information and facilitates communication between the provider and the patient concerning patient.

+ In collaboration with Physician/APP, reinforces health education, counseling, and advice to patients and families on topics such as healthy lifestyle changes. Effectively assesses patient educational needs, prioritizes patient's learning needs, identifies barriers to learning, and uses various educational tools to encourage the non-adherent patient.

+ Evaluates patient's/caller's health status based upon presenting symptoms and follows defined protocols. Determines the urgency for care and directs patient/caller to the most appropriate level of care. Should access not be readily available at the practice, will direct patient/caller to an acceptable alternative venue of care. Seeks Physician/APP advice appropriately.

+ Tracks, reviews, and monitors the follow-up of diagnostic test results to ensure the physician/APP have verified results and patient has been notified. Provides related education, counseling, ongoing review, and monitoring regarding diagnostic tests (i.e., INR results in Anticoagulation Clinic) under the collaboration of the Physician/APP. Assists in the management of patient care between visits.

+ Assists in the management of patient utilization of health care system and provides patient education relative to health care needs, use of community resources and responsibilities of membership in managed care.

+ Completes necessary paperwork, prior authorizations and/or scheduling of appointments for patients who are ordered diagnostic and therapeutic services.

+ Maintains patient records to ensure accurate documentation of the patient's medical treatment.

+ Adheres to Health Insurance Portability and Accountability Act (HIPAA) of 1996.

+ Documents and processes prescription requests and refills per Physician/APP order and or policy.

+ FOR HBO (Hyperbaric Oxygen) Wound Care:

+ For HBO: If requested, will take a 40-hour face to face Introductory Course for Hyperbaric Medicine (recognized by the Undersea and Hyperbaric Medical For HBO: Society) to assist with hyperbaric oxygen therapy treatment as indicated. May monitor hyperbaric dives as requested after completion of required competencies.

+ For HBO: Performs the following testing and treatments: transcutaneous oxygen monitoring testing as ordered and per appropriate policies and procedures; blood glucose monitoring as ordered and indicated by patient diagnosis; and hyperbaric treatments per the patient's treatment plan. Monitors patient during hyperbaric oxygen therapy.

+ For HBO: Educates patient and/or patient's family as to the benefits, risks, contraindications, and safety issues of hyperbaric oxygen therapy prior to treatment.

+ For HBO: Evaluates the patient outcome and meets with the manager and the hyperbaric physician to discuss patient treatment and adjust treatment as necessary.

+ For HBO: Provides assistance in Wound Center operating only within their current clinical licensing scope of practice.
